2025-2026 USJ Pharmacy Prerequisite Course Waiver Request Form

To be counted towards a prerequisite requirement, courses must have been completed within 7 academic years of matriculation (Fall 2019 or later) at a regionally accredited institution. Candidates with prerequisite coursework completed prior to this date must request a waiver. To be considered for a waiver, candidates must submit this form, listing the courses in question and must be able to demonstrate they have been using the content of that course since having completed it, either by taking related upper-level coursework or through work history. Decisions regarding waivers will be made by the Admission Committee on a case-by-case basis. 

Please note, the admissions committee will only consider course waiver request forms for prerequisite coursework completed outside of the 7 academic year deadline if the applicant has completed college-level coursework within the past three (3) years at a regionally-accredited institution. Applicants will be required to upload unofficial transcript(s) to support this request. An unofficial transcript must be provided that shows you have taken coursework within the last three (3) years. Syllabi may be requested on a case-by-case basis.
