2024-2025 USJ PA Studies Program Prerequisite Course Waiver Request Form

To be counted towards a prerequisite requirement, courses must have been completed within 7 academic years of matriculation (Fall 2017 or later) at a regionally accredited institution. Candidates with prerequisite coursework completed prior to this date must request a waiver. To be considered for a waiver, candidates must submit this form, listing the courses in question and must be able to demonstrate they have been using the content of that course since having completed it, either by taking related upper-level coursework or through work history. Decisions regarding waivers will be made by the Admission Committee on a case-by-case basis.  *Transcripts and/or syllabi may be requested on a case-by-case basis.
